Where there is no Psychiatrist: a useful text for those dealing with mental illness in a 3rd world setting

I am a family phyician who teaches introductory Medical Psychology at Hope Africa Medical School in Burundi. This is an excellent introductory book for medical students, nursing students and for others who might deal with mental illlness (social workers, village health workers, pastors). It gives general principles and symptoms and discusses what can cause them. Short case vinettes are often presented. It is very easy for someone who is not familiar with mental illness and substance abuse to understand it, even if their first language is not English. My students read the book, practice interviewing each other and understand it. Difficult subjects like the difference between delerium and dementia are also presented. One chapter deals with mental health issues in various groups like prisoners, refugees, homeless etc. This chapter really brings things home to them and covers things that much larger psychiatric texts may skip. It is socially and clinically very revelant. It would be useful for anyone feeling unprepared to deal with mental illness. I hope that it will soon be translated into Spanish and French.

Dealing with Mental Illness for the Community Health Worker

This book is designed primarily for community health professionals in developing nations. As a longtime owner of Where There is No Doctor, I bought this book hoping it would be as useful and enlightening as "Doctor" was, not only for international situations but even for backcountry situations here in the U.S., or if you are poor and don't have access to medical care here in America.

Physical illness is understood and treated much more readily in international aid and even domestic crises. Most people no matter the culture are much more comfortable dealing with the physically injured than the mentally/emotionally/spiritually injured. People shy away from the mentally ill, not knowing what to do.
Having encountered mentally ill people while in Nigeria in 1996 as well as in various cities in the U.S., I wondered what I might be able to learn by reading this book.

The book begins with an overview of mental illness, including disorders common to all cultures: depression and anxiety, psychoses, special issues with the elderly and children, causation and cultural factors. It then goes on to assessment of a patient, interviews and diagnosis. Treatment options follow, focusing on drug treatments and counseling ("talking treatments").

The next section goes into more depth on clinical problems, such as addressing troublesome behaviors such as violence, aggression, confusion, hearing voices, suicidal tendencies, seizures and fits, postpartum issues, and mental illness among the elderly. It goes on then to look at symptoms such as psychosomatic illness, panic/fear, insomnia, being tired all the time, sexual problems, loss of bodily function, alcoholism and binge drinking, sleeping pill addiction, tobacco and gambling addictions. Next it addresses problems resulting from trauma, such as spouse abuse, rape, or death of a loved one. Problems in childhood and adolescence are looked at, such as childhood development, retardation, ADHD/hyperactivity, child abuse, bed-wetting, and adolescent depression.

One of the most interesting and unique sections of the book is "Mental Health in Other Contexts," such as primary care (focus on prevention rather than cure of disease) and reproductive health... and uniquely, mental illness among prisoners, refugees, in times of disaster, homeless people, street children, and HIV/AIDS. There is also an excellent reminder that healthcare workers must also care for their own mental health.

The author then discusses mental health promotion and advocacy, including creating support groups, prevention of mental retardation, school programs, early identification of illness, prevention of substance abuse, human rights and mental illness, breakdown of relationships, poverty and gender issues.

A special effort is made to give tips on how to adapt the manual for local issues, essential medicines to have on hand, a quick reference guide to medicines for mental illness, and a place where you can write down special resources and contacts for your area in the manual itself. There are flow charts for clinical problem-solving, a glossary of terms, a bibliography and index. Numerous cartoons and illustrations help the readability and engagement with the text.

Overall this is a fantastic resource that was much needed and should be made widely available.

There are some issues that I would like to see expanded in future editions, or perhaps another book:

The paradigm/operating assumption of the book is western biomedicine and western psychology. Although mental illness can have organic causes that are treatable through medication, more recognition needs to be paid to cultural context of mental conditions and working within the cultural paradigms of various cultures.

Different cultures and societies have different definitions of mental illness. For example, being a "were" among the Yoruba of Nigeria, or having susto in Latin America. More attention and recognition should be paid to how western healthcare can successfully collaborate with indigenous healers in the treatment of patients.
And what if you don't have meds? Where There is No Doctor supplied some stopgap solutions when you are really stuck without resources.

Western biomedicine, psychiatry and psychology can learn much from traditional cures and treatments, indigenous knowledge systems, as well as working cooperatively with local healers, spiritual leaders, and other specialists. There are many examples of this in the U.S. in the Indian Health Service, which has a good and still developing record of working with medicine people and healers in caring for patients within traditional systems, including prevention and counseling.

There also needs to be a greater expansion of special issues in times of natural disaster, refugees, etc., including group psychology, mobs, rioting, crime, and devastation. I don't know if there is something like it out there already, but there should be a book specially written just about dealing with mental health in large scale disasters, such as wars, tsunami, earthquakes, epidemics, and societal collapse, in developed or developing countries. The lessons about mental health have yet to be distilled from Katrina in New Orleans, the Asian Tsunami, and the still unfolding tragedy in Haiti.

About the Author
Larry Neal is Emeritus Professor of Economics at the University of Illinois, Urbana-Champaign and Research Associate of the National Bureau of Economic Research, and was Visiting Professor at the London School of Economics and Political Science from 2006 to 2014. Specialising in financial history and European economies, he is author of The Rise of Financial Capitalism: International Capital Markets in the Age of Reason (Cambridge, 1991). His recent work includes editing the two Cambridge History of Capitalism volumes - The Rise of Capitalism: From Ancient Origins to 1848 and The Spread of Capitalism: From 1848 to the Present (Cambridge, 2014), and authoring 'I Am Not Master of Events': The Speculations of John Law and Lord Londonderry in the Mississippi and South Sea Bubbles (2012). For many years he was editor of the journal Explorations in Economic History, which now awards an annual prize in his name for the best article published each year. He is past President of the Economic History Association and the Business History Conference and was the Founding Director of the European Union Center at the University of Illinois in 1998, which awards an annual prize in his name for an outstanding book on the European Union. He has been the recipient of fellowships from the Alexander von Humboldt Stiftung (1982), when he visited the University of Paderborn in Germany, as well as the John Simon Guggenheim Foundation and the Fulbright Commission (1996-7), when he visited the London School of Economics and Political Science. He was a visiting professor at the California Institute of Technology in 1998, and the University of California, Los Angeles in 2008, and was named one of the first ten Fellows of the Cliometric Society in 2010.

Informative, and Frustrating

Lots of detail, events, characters, and policy here. Not well written, though. Many passive sentences, which confuse or omit the subjects / focus of the story. Also, topics arise without explanation only to arise later in passing detail. Still, I learned much about the evolution of financial markets and assets. Worth the read overall.

It is not claimed that the following pages contain any new ideas. They were inspired by the writings of Tolstoy, who was the first, and in fact the only, author of my acquaintance who ever seemed to me to place the doctrine of non-resistance upon a substantial basis. After reading
Tolstoy I determined to make a careful study of the subject, but on a thorough search of book stores and libraries could find next to nothing dealing with the question, while the shelves were crowded with literature extolling the glories of war and the beneficence of patriotism.

The first part of this volume which deals with the state is very fragmentary, and in no wise so complete as can be found in many other volumes, but in the portion which deals with crime and punishment, I have found a much newer field, and one which has generally been dis-
cussed by those who have little practical knowledge of the machinery of courts of justice.

It has been my purpose to state the reasons which appeal to me in support of the doctrine of non-resistance, rather than to give authorities to sustain the theories advanced. Still, I believe that the student who is interested in the subject of criminology, and wishes to carefully investigate crime and punishment, will find that most of the great historians, philosophers, and thinkers will amply corroborate the views herein set forth, as to the cause of crime, and the evil and unsatisfactory results of punishment.

Clarence Seward Darrow (April 18, 1857 – March 13, 1938) was an American lawyer and leading member of the American Civil Liberties Union, best known for defending teenage thrill killers Leopold and Loeb in their trial for murdering 14-year-old Robert "Bobby" Franks (1924) and defending John T. Scopes in the Scopes Trial (1925), in which he opposed William Jennings Bryan (statesman, noted orator, and 3-time presidential candidate). Called a "sophisticated country lawyer", he remains notable for his wit and agnosticism, which marked him as one of the most famous American lawyers and civil libertarians.

Well-considered criticism of "criminal justice"

Now best known for his defense of a teacher's right to teach evolution in the "Scopes Monkey Trial," Clarence Darrow devoted much of his life in the courtroom to an expert public criticism of the notion of "criminal justice" and of the penal system in the U.S. Rather than focus his energy on "winning" the cases of his clients, he deconstructed conventional conceptions of "blame," "guilt," et al., arguing that punishment itself contributes to the oppressive conditions that produce "crime." Interestingly, he often "won" these cases on the merit of his theory, in spite of giving scant attention to the particulars of the individual cases.
This compendium of Darrow's work is an excellent (and extremely well-written) cross-section of his ideas -- a must-read for anyone interested in law, juridical theory, anarchism, etc.

Excellent Well-Written Book That Is As Relevant In The Present As In The Past.

This is an excellent well-written book that is as relevant today as it was when Clarence Darrow originally wrote it over 100 years ago. Carol Moore wrote an excellent Introduction to the book in the Loompanics print version of this book. Clarence Darrow was one of the most successful defenders of liberty and justice. He was a compassionate man who had deep and genuine sympathy for the ignorant, poor, and oppressed people. He claimed that all states, even democracies, wield their power to benefit the rich and powerful and to keep the poor and oppressed powerless. He opposed the death penalty claiming that he felt it was nothing but legalized murder. Clarence Darrow said it is a strange code of negative ethics that says a man should be punished for an evil act and not be be rewarded for a virtuous one. Men's lives are a strange mixture of good and evil. No life is wholly good, and no life is wholly bad. A life of great virtues may here and there be interspersed with an evil act. The law picks out the evil and ignores the good.
