Where there is no Psychiatrist: a useful text for those dealing with mental illness in a 3rd world setting

I am a family phyician who teaches introductory Medical Psychology at Hope Africa Medical School in Burundi. This is an excellent introductory book for medical students, nursing students and for others who might deal with mental illlness (social workers, village health workers, pastors). It gives general principles and symptoms and discusses what can cause them. Short case vinettes are often presented. It is very easy for someone who is not familiar with mental illness and substance abuse to understand it, even if their first language is not English. My students read the book, practice interviewing each other and understand it. Difficult subjects like the difference between delerium and dementia are also presented. One chapter deals with mental health issues in various groups like prisoners, refugees, homeless etc. This chapter really brings things home to them and covers things that much larger psychiatric texts may skip. It is socially and clinically very revelant. It would be useful for anyone feeling unprepared to deal with mental illness. I hope that it will soon be translated into Spanish and French.

A good accompaniment to Where There is No Doctor and Where There is No Dentist, both essentials for the rural health care worker. There is emphasis on non-drug related treatments for mental illness but drug treatments are also mentioned.

Dealing with Mental Illness for the Community Health Worker

This book is designed primarily for community health professionals in developing nations. As a longtime owner of Where There is No Doctor, I bought this book hoping it would be as useful and enlightening as "Doctor" was, not only for international situations but even for backcountry situations here in the U.S., or if you are poor and don't have access to medical care here in America.

Physical illness is understood and treated much more readily in international aid and even domestic crises. Most people no matter the culture are much more comfortable dealing with the physically injured than the mentally/emotionally/spiritually injured. People shy away from the mentally ill, not knowing what to do.
Having encountered mentally ill people while in Nigeria in 1996 as well as in various cities in the U.S., I wondered what I might be able to learn by reading this book.

The book begins with an overview of mental illness, including disorders common to all cultures: depression and anxiety, psychoses, special issues with the elderly and children, causation and cultural factors. It then goes on to assessment of a patient, interviews and diagnosis. Treatment options follow, focusing on drug treatments and counseling ("talking treatments").

The next section goes into more depth on clinical problems, such as addressing troublesome behaviors such as violence, aggression, confusion, hearing voices, suicidal tendencies, seizures and fits, postpartum issues, and mental illness among the elderly. It goes on then to look at symptoms such as psychosomatic illness, panic/fear, insomnia, being tired all the time, sexual problems, loss of bodily function, alcoholism and binge drinking, sleeping pill addiction, tobacco and gambling addictions. Next it addresses problems resulting from trauma, such as spouse abuse, rape, or death of a loved one. Problems in childhood and adolescence are looked at, such as childhood development, retardation, ADHD/hyperactivity, child abuse, bed-wetting, and adolescent depression.

One of the most interesting and unique sections of the book is "Mental Health in Other Contexts," such as primary care (focus on prevention rather than cure of disease) and reproductive health... and uniquely, mental illness among prisoners, refugees, in times of disaster, homeless people, street children, and HIV/AIDS. There is also an excellent reminder that healthcare workers must also care for their own mental health.

The author then discusses mental health promotion and advocacy, including creating support groups, prevention of mental retardation, school programs, early identification of illness, prevention of substance abuse, human rights and mental illness, breakdown of relationships, poverty and gender issues.

A special effort is made to give tips on how to adapt the manual for local issues, essential medicines to have on hand, a quick reference guide to medicines for mental illness, and a place where you can write down special resources and contacts for your area in the manual itself. There are flow charts for clinical problem-solving, a glossary of terms, a bibliography and index. Numerous cartoons and illustrations help the readability and engagement with the text.

Overall this is a fantastic resource that was much needed and should be made widely available.

There are some issues that I would like to see expanded in future editions, or perhaps another book:

The paradigm/operating assumption of the book is western biomedicine and western psychology. Although mental illness can have organic causes that are treatable through medication, more recognition needs to be paid to cultural context of mental conditions and working within the cultural paradigms of various cultures.

Different cultures and societies have different definitions of mental illness. For example, being a "were" among the Yoruba of Nigeria, or having susto in Latin America. More attention and recognition should be paid to how western healthcare can successfully collaborate with indigenous healers in the treatment of patients.
And what if you don't have meds? Where There is No Doctor supplied some stopgap solutions when you are really stuck without resources.

Western biomedicine, psychiatry and psychology can learn much from traditional cures and treatments, indigenous knowledge systems, as well as working cooperatively with local healers, spiritual leaders, and other specialists. There are many examples of this in the U.S. in the Indian Health Service, which has a good and still developing record of working with medicine people and healers in caring for patients within traditional systems, including prevention and counseling.

There also needs to be a greater expansion of special issues in times of natural disaster, refugees, etc., including group psychology, mobs, rioting, crime, and devastation. I don't know if there is something like it out there already, but there should be a book specially written just about dealing with mental health in large scale disasters, such as wars, tsunami, earthquakes, epidemics, and societal collapse, in developed or developing countries. The lessons about mental health have yet to be distilled from Katrina in New Orleans, the Asian Tsunami, and the still unfolding tragedy in Haiti.
